FINANCING
Payment Options
For your convenience, we accept cash, personal cheque, Visa, and Mastercard. Payment for the initial examination and the orthodontic records are expected at the time services are performed.
For the orthodontic treatment interest-free payment plans are available. As the patient (or a parent of the patient), you authorize treatment and you are solely responsible for payment of all fees. If your insurance coverage includes orthodontic treatment, documentation will be provided to you so that you will receive your maximum benefits.
Financial Information
-
Once treatment begins, you will be given the necessary forms to send to your insurance company to reimburse you. In order to preserve the best treatment options for our patients, the British Columbia Association of Orthodontists policy recommends orthodontists do not accept payment directly from insurance companies, so we will bill you directly.
-
The monthly payments do not reflect the work done in a particular month, but merely are a way to divide the fee.
-
Treatment times differ from patient to patient and the number of payments will not increase or decrease, regardless of the length of treatment, unless there is a lack of patient cooperation.
-
Missed appointments and extension of treatment time due to lack of cooperation may result in increased fees.
-
The quoted fee includes all services rendered in our office, except for the replacement of lost or broken appliances.
-
The fee does not include regular dental check-ups or dental work performed outside our office.
-
If services are terminated for any reason before the completion of treatment, the account will be adjusted and a just settlement determined based on the amount of treatment completed. Please be aware that approximately a third of the total treatment fee is incurred at the time of initial placement of appliances.
INSURANCE
-
Orthodontic insurance coverage is not the same as general dental insurance coverage. Please contact your insurance representative to determine whether or not you have orthodontic coverage.
-
If you do have orthodontic coverage, we will submit a Pre-Determination form to your insurance carrier. Your insurance carrier will advise you directly, in writing, as to coverage and benefits once the Pre-Determination form is submitted.
-
No dental codes are required for any orthodontic claim submissions.
-
We do not accept payment from the insurance carrier directly to our office, as we are a specialty office, direct billing is not available.
-
To help us with your insurance forms, please complete the insurance portion on our questionnaire. We will send a Pre-Determination to your insurance provider after the Final Consultation appointment to verify your insurance coverage.
